SHORT TERM MEDICAL INSURANCE
|
What is short term medical insurance?
Short term medical is a temporary plan of insurance designed for people
who are between permanent health plans. Whether you are a graduating
student, between jobs, laid off, or waiting for your employer's health
coverage to begin, a Short term medical plan provides valuable insurance
protection.
How does the plan work?
You choose the deductible and Rate of payment option that meets your
needs. Deductible choices are $250, 500, 1,000, and 2500. Rate of payment
options are: 80/20 or 50/50. After deductible is met, benefits are paid at
the chosen rate until $5,000 has been reached. At that point, benefits are
paid at 100%, to the maximum of $2,000,000 for each covered person.
Who is eligible for this plan?*
Healthy individuals between the ages of 15 days and age 64 and 11 months
are eligible. Children up to age 20 (24 if full-time student) may apply
for coverage as dependents on their parent's plan.
*This plan is not
available to non-US citizens, and residents of Alaska, Hawaii, and New
York.
What are the benefits?
Any expense incurred by an insured for services, treatment or supplies
prescribed by a Physician as the result of Sickness or Injury; for
medically necessary care; and incurred while this plan is in force. A
covered expense does not include any charge in excess of the reasonable
and customary charge.
